Ação lúdico terapêutica no Hospital de Clínicas de Porto Alegre em perspectiva institucional emancipatória

Abstract

O presente estudo objetivou compreender, à luz da pedagogia emancipatória proposta por Paulo Freire, as dinâmicas processuais em contexto hospitalar oriundas das ações lúdicas desenvolvidas pelo Serviço de Recreação Terapêutica do Hospital de Clínicas de Porto Alegre - HCPA. Um serviço assistencial que é pioneiro no Brasil no oferecimento de atividades recreativas e ocupacionais como parte do tratamento de pacientes clínicos, cirúrgicos e psiquiátricos de todas as faixas etárias em nível preventivo, curativo e paliativo. Procurou-se analisar quais são e como estão representadas as dinâmicas processuais subjacentes a esta atividade e que, cotidianamente, perpassam as ações e relações nesta Instituição de saúde sob a ótica freiriana. Para tanto, utilizou-se a pesquisa qualitativa focada no Estudo de Caso, sendo usada como técnica principal a entrevista semi-estruturada realizada em pacientes, familiares, acadêmicos e profissionais de saúde envolvidos com a ação em questão, complementada pela análise da realidade atual e registros históricos do Serviço de Recreação Terapêutica/HCPA. Os dados coletados foram utilizados após múltiplas leituras e interpretações baseadas em categorias selecionadas no extenso repertório de Paulo Freire: O diálogo, pontuando as igualdades e desigualdade em saúde, a participação crítica e a autonomia, sinalizando a existência de movimentos libertadores capazes de validar as rotinas, padrões e papéis exercidos pelos sujeitos neste contexto. Dentre os resultados encontrados no cenário analítico deste estudo, destaca-se a existência de discursos hegemônicos cristalizados, bem como a necessidade de que se estabeleça durante a assistência lúdica uma escuta criteriosa, responsável e democrática que acolha as demandas individuais e coletivas na busca de um cuidado integral, integrado, resolutivo e humanizado. Um cuidado que valorize o compartilhamento de saberes e fazeres de todos os envolvidos no processo saúde-doença-intervencão.

This study had as a goal to understand, in the light of the emancipatory pedagogy proposed by Paulo Freire, the process dynamics in a hospital context coming from ludic activities developed by the Therapeutic Recreation Service at the Hospital de Clínicas de Porto Alegre - HCPA. It is a social service that is the Brazilian pioneer in providing recreational and occupational activities as part of treatment to the clinical, surgical and psychiatric patients of all ages in the preventive, curative and palliative levels. There is an effort to analyze the nature of the process dynamics which underlies these activities and pervades everyday actions and relationships in this institution from the perspective of Paulo Freire, and how it is represented. For this purpose, a qualitative research focused on the case study was used, having as its main technique the semi-structured interview with patients, family, scholars and health professionals involved with the action observed, complemented by an analysis of the current reality and historical records of the Department of Therapeutic Recreation / HCPA. The data collected were used after multiple readings and interpretations based on selected categories from the wide repertoire of Paulo Freire: dialogue, pointing the equalities and inequality in health, critical participation and autonomy, indicating the existence of liberation movements which can validate the routines, patterns and roles of the subjects in this context. Among the findings in the analytical stage of this study, the existence of crystallized hegemonic discourses, as well as the need to establish, during the ludic assistance, a careful, responsible and democratic listening that embraces the individual and collective demands in pursuit of an integrated, resolute and humanized comprehensive care, which values the sharing of knowledge and actions of everyone involved in the health-diseasehospitalization process.
